This is an Adobe form file. This file is opened by Adobe Acrobat Professional.

There is a trick though. You don't open this file alone. You must first open the PDF that was used to create this file. The PDF was used as a form and the FDF stores the data for the form. That way users can open the main form and enter data and still leave the main form intact for the next person. After you open the main PDF then you have to apply the form or import the form. I think that is on the Advanced menu of something like that. Under that I believe is a Forms sub menu.
